The record of policyholders and beneficiaries of health, pension and disability insurance

Employers, legal entities and natural persons are obliged to register an employee within the five days from the day of signing the labor contract, as follows:
- At the National employment Service (Form E1 and E3)
- At the Fund for Pension and Disability Insurance (Form M1 and M2).

For the registry record of policyholders and beneficiaries of health, pension and disability insurance, registration data records are submitted to the branch office.
The register of entrepreneurs and founders is kept by the Fund of independent business.


1. Registration with the Health Insurance Fund:
To the Republic Institute for Health Insurance the following documentation is submitted:
- Insurance application (Forms: M-2, M-3A, – form are self-copy forms and can be bought at bookstores)
- Decision on registration with the Serbian Business Registers Agency
- Form ROD 1 – Registration of the commissioning the business activity,
- Old health card and blank form for new health card
- The list of issued health cards on the Form ZOK

2. Registration with the Fund for Pension and Disability Insurance 

Founders and entrepreneurs submit to the main branch office of the Fund the insurance application (Forms M1 or M1K, M2, M3A). The form can be obtained at the Fund.
The following documentation shall be submitted with the application:
- Decision on registration with the Serbian Business Registers Agency,
- Registration of the commissioning the business activity, Form ROD 1,
- Memorandum on Association,
- Decision on tax identification number
- Contract with the bank.
